OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees, in that employees were exposed to the hazard of being crushed:    WORKPLACE: The employer did not ensure that employees were clear of danger zones while operating the International Closed Door Baler (machine), Model CD-4843-8-740-60 & Serial Number 0812407.    On or about March 21, 2015, one of employees' hand got caught in between the locking mechanism and the baler door of the machine while performing the above tasks, and the employee sustained serious injuries to two (2) fingers from the incident.
