29 CFR 1926.416(a)(1): Employees were permitted to work in proximity to electric power circuits and were not protected against electric shock by de-energizing and grounding the circuits or effectively guarding the circuits by insulation or other means:     (a). Worksite located at 4723 N. Highland, Kansas City, Missouri- Employee was exposed to a shock or electrocution hazard in that the employee had placed an aluminum extension ladder in direct proximity of an energized overhead power line.

29 CFR 1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems:   (a). Worksite located at 1609 NE Vivion Road, Kansas City, Missouri- Employee was observed working and walking on the roof of the building  without the use of any means of fall protection.

Worksite located at 1609 NE Vivion Road, Kansas City, Missouri- Employee on the roof's edge had not been trained by the employer on the recognition and avoidance of fall hazards.
