The propane tank was not secured by the locator pin; on or about 3/27/15.    Note:  The employer is required to submit abatement certification for this item in accordance with 29 CFR 1903.19.

29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks:   a)  Worksite, 25 Corporate Dr. Hauppauge NY, 11749 - Employees operating a Baldor belt sander were not protected from the ingoing nip points between the belt and the pulley; on or about 3/27/15.  b)  Worksite, 25 Corporate Dr. Hauppauge NY, 11749 - Employees operating a Duplex belt sander were not protected from the ingoing nip points between the belt and the pulley; on or about 3/27/15.   Note:  The employer is required to submit abatement certification for this item in accordance with 29 CFR 1903.19.
