29 CFR 1910.1030(c)(1)(i): The employer having employee(s) with occupational exposure did not establish a written Exposure Control Plan designed to eliminate or minimize employee exposure:    Establishment - The employer had not developed a written Exposure Control Plan that was customized for the facility.  All site-specific sections were left blank.  On or about March 26, 2015, the employer's written Exposure Control Plan (ECP) was not adequate in that the program was not specific to this employer who had employees working at this site.   The employer's program consisted of a sample program with the fill-in-the blank sections not completed.      Sava Senior Care (dba Belmont Lodge Health Care Center) was previously cited for a violation of this occupational safety and health standard or its equivalent standard, which was contained in OSHA inspection number 109540, citation number 1, item number 1 and was affirmed as a final order on February 8, 2012, with respect to a workplace located at 1601 Constitution Road in Pueblo, Colorado.
