Employee was exposed to eye and face injuries when operating a pneumatic nail gun while not wearing any eye and or face protection.      Condition occurred on or about 03/31/15.

29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet or more above the lower levels was not protected by guardrail systems, safety net systems, personal fall arrest systems, nor was an alternative fall protection plan which meets the requirements of 29 CFR 1926.502(k) developed and implemented:      a) 247 Washington Street, Berkeley Heights, NJ 07922. Employees were exposed to fall hazards when they were installing new roofing materials on the roof of a newly constructed two story home with no fall protection systems in place.       Condition occurred on or about 03/31/15.

29 CFR 1926.1053(b)(16): Portable ladders with structural defects, such as, but not limited to, broken or missing rungs, cleats, or steps, broken or split rails, corroded components, or other faulty or defective components, were not either immediately marked in a manner that readily identified them as defective, or tagged with Do Not Use or similar language, and were not withdrawn from service until repaired:     a) 247 Washington Street, Berkeley Heights, NJ 07922. Employees were exposed to fall hazards when using a damaged portable ladder to ascend or descend the roof when the safety shoes were missing and the rungs were damaged.       Condition occurred on or about 03/31/15.
