29 CFR 1926.652(a)(1): Each employee in an excavation was not protected from cave-ins by an adequate protective system designed in accordance with paragraph (b) or (c) of this section:  On or about March 31, 2015, employees were exposed to cave-in hazards, when inside a trench, with an inadequate protective system, consisting of benching in a class C soil, measuring approximately 6 feet 6inches in depth, 7 feet in width, and 10 feet in length while painting a line at the bottom of the trench to mark a water line.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:  On or about March 31, 2013, employees were exposed to a fall hazard of 6 feet 6 inches in height to unpaved ground level below when using a portable ladder that was not 3 feet above the upper landing surface to access and egress a trench.
