Crossbraces were used as a means of access.    (a) Pooler construction site, on or about and at times preceding to March 31, 2015, tubular welded frame scaffold was not equipped with an access ladder for employees laying brick 15 feet above the ground.

(a) Pooler construction site, on or about and at times preceding to March 31, 2015, the employer did not provide training about hazardous chemicals for employees who were exposed to chemicals such as, but not limited to silica from cutting bricks and Portland cement in mortar, resulting in lung and skin hazards.
