29 CFR 1910.23(c)(1): Open-sided floor(s) or platform(s) 4 feet (1.22 m) or more above the adjacent floor or ground level were not guarded by standard railings (or the equivalent as specified in 29 CFR 1910.23(e)(3)(i) through (v)), on all open sides:  a)  On or about April 1, 2015 and at times prior to, three employees were observed walking/working on the flat roof of a building.  The employee were engaged in cleaning and replacing activities.  The employees were not utilizing fall protection, exposing the employees to a fall hazard of about 10 feet 6 inches to the ground below.
