29 CFR 1926.501(b)(13):  Each employee engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system:    On March 11, 2015, and at times prior thereto, at the location 5244 West Loop 250 N, Midland, Texas 79707, employees were removing old metal roofing to install new asphalt shingles.  Employees were exposed to a fall more than six (6) feet to the lower level.

29 CFR 1926.1053(b)(1):   Portable ladders that are used to gain access to an upper landing surface, the ladder was not secured at its top to a rigid support that will not deflect, and a grasping device, such as a grabrail, to assist employees in mounting and dismounting the ladder:    On March 11, 2015, and at times prior thereto, at the location 5244 West Loop 250 N, Midland, Texas 79707, employees were accessing the roof by way of ladder that was not secured to the roof.  Employees were exposed to a fall more than six (6) feet to the lower level.
