29 CFR  1910.147(c)(6)(i): The employer did not conduct a periodic inspection of the energy control procedure at least annually to ensure that the procedure and the requirements of this standard are being followed.  a) On or about March 30, 2015 or prior thereto: Quality Truss & Lumber, Inc. had not conducted a periodic inspection of energy control procedures in the workplace.  Lockout is performed on such equipment as the 240 Volt Spida 2000 Saws.  Temporary labor of Extreme Staffing of Idaho utilize these saws during truss manufacturing.  Abatement certification and documentation is required for this item.

29 CFR 1910.213(h)(1):   The sides of the lower exposed portion of the blade were not guarded to the full diameter of the blade by a device that will automatically adjust itself to the thickness of the stock and remain in contact with stock being cut to give maximum protection possible for the operation being performed.  a) On or about March 30, 2015 or prior thereto:  The lower portion of Spida Saw #1 was not guarded to prevent contact with the saw during truss manufacturing.  b) On or about March 30, 2015 or prior thereto:  The lower portion of Spida Saw #3 was not guarded to prevent contact with the saw during truss manufacturing.  Abatement certification and documentation is required for this item.
