29 CFR 1926.102(a)(1): Eye and face protective equipment were not used when machines or operations presented potential eye or face injury from physical, chemical, or radiation agents.    a) At the jobsite, on or about March 31, 2015, employees were performing wood framing activities that included operating portable electric hand tools such as pneumatic nail guns, and circular saws without the use of eye protection.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):     a) At the jobsite, on or about March 31, 2015, employees working on a second working level while performing wood framing were exposed to open floor joists and exposed leading edges without fall protection provided, exposing employees to fall hazards of 11.4 feet up to onto the ground (outside the structure) or concrete floor (inside the structure).
