29 CFR 1910.212(a)(1):  Machine guarding was not provided to protect operator(s) and other employees from hazard(s) created by in going nip points, and unguarded conveyor belts:           (a) Cincinnati OD Grinder, Model 16x17, Serial No. 1P5F1Y-4: On or about April 1, 2015 an employee dressing the left side of the grinding wheel was exposed an amputation hazard created between the part being ground and the grinding wheel.  The grinding wheel was being dressed free hand and was not moved away from the part creating an ingoing nip point.
