29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet (1.8 m) or more above lower levels was not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employees provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):  a) Airtson Ct., Lot 90, Weatherstone Crossing, Felton, DE - On or about April 1, 2015, the employer did not provide fall protection to employees conducting roofing activities, working approximately 20 feet above the grade, exposing employees to falls.        Woodpecker Home Improvements, LLC d/b/a Woodpecker Construction Company was previously cited for a violation of this occupational safety and health standard, which was contained in OSHA inspection number 940583, citation number 1, item number 1 and was affirmed as a final order on 10/21/2013, with respect to a workplace located at Lot 27, Huntfield Drive, Smyrna, DE.   Woodpecker Home Improvements, LLC d/b/a Woodpecker Construction Company was previously cited for a violation of this occupational safety and health standard, which was contained in OSHA inspection number 316184076, citation number 1, item number 1 and was affirmed as a final order on 05/11/2012, with respect to a workplace located at Roosa Road, Milford, DE.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:  a) Airtson Ct., Lot 90, Weatherstone Crossing, Felton, DE - On or about April 1, 2015, the employer did not make sure employees used ladders that extended at least 3 feet above the upper landing for roof access, exposing employees to falls.
