29 CFR 1910.303(g)(1): Sufficient access and working space was not provided and maintained about all electric equipment (operating at 600 volts, nominal, or less to ground) to permit ready and safe operation and maintenance of such equipment: a) In Production department - The employer failed to ensure that employees were provided adequate access to the 110 V main electrical panel that was used for lights and switches for the entire building. The panel was blocked with carts loaded with painted parts and employees were exposed to unsafe operation and maintenance in the event of an emergency.

29 CFR 1910.134(c)(1): A written respiratory protection program that included the provisions in 29 CFR 1910.134(c)(1)(i) - (ix) with worksite specific procedures was not established and implemented for required respirator use: a) In paint booth - The employer failed to provide the respiratory protection program to employees who were required to use the NIOSH 3M - 60921 Dual Cartridges respirators while painting parts using lacquer thinner, lacquer sealer and premium plus. Employees were exposed to irritation of the upper respiratory system.

29 CFR 1910.1200(h)(1): Employees were not provided effective information and training on hazardous chemicals in their work area at the time of their initial assignment and whenever a new hazard that the employees had not been previously trained about was introduced into their work area: a) Throughout facility - The employees were utilizing chemicals and painting products such as, but not limited to, Lacquer Thinner, Hi-Bild Lacquer Sealer, Behr Premium Plus and Propane gas when they were not provided with information and training in regards to hazards of chemicals that they were working with. Employees were exposed to eye, skin and respiratory tract irritations.
