The portable eyewash facility provided and required to be used during battery fluid level checks was not set up for use and made immediately accessible in the event of an emergency. While operating a standard, sit-down type electrically powered forklift truck the operator failed to: 1/ trail the load when the load was higher than his ability to see over it 2/ watch for pedestrian traffic 3/ go slow down enough to prevent product from flying off the pallet 4/ sound the horn when approaching an aisle/intersection Observed on/or about 04/01/2014.
