The air compressor was not plugged into a ground-fault circuit interrupter.  The company was not utilizing an assured equipment grounding program.

Employees had not recieved electrical safety training on items including but not limited to extesion cord inspection/safety requirements and ground fault circuit interrupters.   B.)  Pitzulo Brothers Company, Inc., 240 North Hermitage Road, Hermitage, PA 16148, on or about April 1, 2015 and times prior thereto:  Employees were utilizing personal protective equipment including, but not limited to, safety glasses.  Employees were also exposed to potential head injury hazards and were not wearing hard harts. Employees did not have personal protective equipment (PPE) training on items that included but were not limited to the required use of of PPE, when to use the PPE, and the care and mainteance of the PPE.
