29 CFR 1926.451(b)(1): Each platform on all working levels of scaffolding was not fully planked or decked:   (a)  On or about 04/02/15, at the Danny Stefanos site, located on 830 South Avenue, in Rochester, New York, a supported scaffold in use for masonry work did not have all working levels (at 20 feet above ground level) fully planked.  NO ABATEMENT CERTIFICATION REQUIRED

29 CFR 1926.451(g)(1)(vi): Each employee performing overhand bricklaying operations from a supported scaffold was not protected from falling from all open sides and ends of the scaffold (except at the side next to the wall being laid) by the use of a personal fall arrest system or guardrail system (with minimum 200 pound toprail capacity).    (a)  On or about 04/02/15, at the Danny Stefanos site, located on 830 South Avenue, in Rochester, New York, workers laying brick along the exterior wall were not protected with fall protection at the open end along the east side of the scaffold.   NO ABATEMENT CERTIFICATION REQUIRED
