29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks:    a) On or about April 20, 2015, the employer failed to ensure that the #4130 CNC machine with serial number 125163 was equipped with interlocks to prohibit employees from opening the doors while the equipment was in operation.     b) On or about April 20, 2015, the employer failed to ensure that the #4150 CNC machine with serial number 257908 was equipped with interlocks to prohibit employees from opening the doors while the equipment was in operation.     c) On or about April 20, 2015, the employer failed to ensure that the #4129 CNC machine with serial number 116685 was equipped with interlocks to prohibit employees from opening the doors while the equipment was in operation.     d) On or about April 20, 2015, the employer failed to ensure that the Fadal CNC machine with serial number 9906884 was equipped with interlocks to prohibit employees from opening the doors while the equipment was in operation.     e) On or about April 20, 2015, the employer failed to ensure that the #3157 CNC machine with serial number 246846 was equipped with interlocks to prohibit employees from opening the doors while the equipment was in operation.     f) On or about April 20, 2015, the employer failed to ensure that the #2158 vertical CNC machine with serial number v600054 was equipped with interlocks to prohibit employees from opening the doors while the equipment was in operation.     g) On or about April 20, 2015, the employer failed to ensure that the Mitsubishi #3109 vertical CNC machine with serial number cv6298 was equipped with interlocks to prohibit employees from opening the doors while the equipment was in operation.     h) On or about April 20, 2015, the employer failed to ensure that the vertical milling machine #1102 with serial number 150/14/54 was equipped with a chuck and chip guard to protect employee from metal particles and coolant.     i) On or about April 20, 2015, the employer failed to ensure that the vertical milling machine #1104 with serial number 542b-120 was equipped with a chuck and chip guard to protect employee from metal particles and coolant.    j)  On or about April 20, 2015, the employer failed to ensure that the Fermat vertical milling machine with serial number F-01-0167 was equipped with interlocks to prohibit employees from opening the doors of the operator station while the equipment was in operation. The operator station when effectively interlocked also protects the employee from the movement and pinch points of the equipment tables.     k) On or about April 20, 2015, the employer failed to ensure that the Lodge Shipley Lathe located in bay 4 was equipped with a chuck and chip guard to protect employees.    l) On or about April 20, 2015, the employer failed to ensure that the Slovak TOS #33 lathe located in bay 4 was equipped with a chuck and chip guard to protect employees.
