29 CFR 1926.501(b)(14): Each employee working on, at, above, or near wall openings (including those with chutes attached), where the outside bottom edge of the wall opening was 6 feet (1.8 m) or more above lower levels and the inside bottom edge of the wall opening is less than 39 inches (1.0 M) above the walking/working surface, is not protected from falling by the use of a guardrail system, a safety net system, or a personal fall arrest system: a. Building B- An employee standing directly in front of a wall opening was not protected from a fall of approximately 30 feet where fall protection was not utilized.

29 CFR 1926.501(c)(3): The employer did not barricade the area to which objects could fall, did not prohibit employees from entering the barricaded area, and did not keep objects that could fall far enough away from the edge of a higher level so that those objects would not go over the edge if they were accidentally displaced: a. Building B- Employees working from the ground were not protected from overhead struck by hazards where a foreman dropped a fish tape approximately 30 feet to the ground, over the building exit. The building exit and area to where the fish tape was dropped was not barricaded.
