29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks:  At the time of inspection, during rebar fabrication, the employer failed to provide machine guarding to machinery in the facility, including, but not limited to:  1) The RMS Spiral-Fab, unknown serial number, had a foot pedal located directly under the point of operation, and it did not have functioning interlocks on the back door where access is gained to lubricate the gears and chains, and  2)  Minimax 80 V US, serial # MM 83049 V US, did not have functioning interlocks on the front and back doors. The front doors are opened when the machine is put in manual mode to feed a new coil of rebar through, and the back door is opened to service the motor, hydraulics, and gears inside.
