29 CFR 1926.102(a)(1): Eye and face protective equipment were not used when machines or operations presented potential eye or face injury:      (a)  Enterprise Exteriors, LLC at 350 W. 114th Ave., Northglenn, CO 80234:  On and before March 26, 2015, the exposing employer did not ensure that employees were provided eye protection when power tools such as pneumatic nail guns were used during roofing operations.  Condition exposed employees to the hazard of projectiles to the eyes.

29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet (1.8 m) or more above lower levels was not protected by guardrail systems, safety net system, or personal fall arrest system, nor was an alternative fall protection measure provided:      (a)    Enterprise Exteriors, LLC at 350 W. 114th Ave., Northglenn, CO 80234:  On and before March 26, 2015, the exposing employer did not ensure each employee on a walking/working surface having an unprotected side or edge which was six feet (1.8m) or more above a lower level was protected from falling by the use of a guardrail system, a safety net system, or personal fall arrest system in that approximately twelve employees were engaged in tearing off an old roof and applying a new roof on a single story apartment complex. This condition exposed the employees to fall hazards of more than 6 feet and up to approximately 18 feet.

29 CFR 1926.502(d)(16):  Personal fall arrest systems, when stopping a fall, shall be rigged such that employees can neither free fall more than 6 feet, nor contact any lower level:    (a)   Enterprise Exteriors, LLC at 350 W. 114th Ave., Northglenn, CO 80234:  On and before March 26, 2015, the exposing employer did not ensure that the personal fall arrest systems used by employees limited the fall distance to 6 feet, nor prevented contact with the lower level.  At least three employees engaged in roofing activities were wearing fall harnesses attached to anchored vertical lifelines; however, their rope grabs and lanyards were not properly adjusted so that fall protection systems did not provide adequate fall protection.  Conditions resulted in employees exposed to fall hazards of greater than 6 feet, and with high potential of contacting the ground below at approximately 9 or 18 feet below.
