29 CFR 1926.451(d)(16): In addition to the normal operating brake, suspension scaffold power-operated hoists and manually operated hoists shall have a braking device or locking pawl which engages automatically when a hoist makes either of the following uncontrolled movements: an instantaneous change in momentum or an accelerated overspeed:    a) On or about April 7, 2015 - at a jobsite located at 6157 S. Evans Ave., Chicago, IL. - employees were performing masonry work from a two point suspension scaffold that was not equipped with a braking device or locking pawl which engages automatically, thereby exposing employees to fall hazards of approximately twenty five (25) to thirty (30) feet.

Guardrail systems shall be installed before the scaffold is released for use by employees other than erection/dismantling crews:    a) On or about April 7, 2015 - at a jobsite located at 6157 S. 29 CFR 1926.501(b)(3): "Hoist areas." Each employee in a hoist area shall be protected from falling 6 feet (1.8 m) or more to lower levels by guardrail systems or personal fall arrest systems. If guardrail systems, [or chain, gate, or guardrail] or portions thereof, are removed to facilitate the hoisting operation (e.g., during landing of materials), and an employee must lean through the access opening or out over the edge of the access opening (to receive or guide equipment and materials, for example), that employee shall be protected from fall hazards by a personal fall arrest system:    a) On or about April 7, 2015 - at a jobsite located at 6157 S. Evans Ave., Chicago, IL. - an employee working on the roofing of a building performing masonry work was exposed to falls due to the presence of unprotected sides/edges, thereby exposing the employee to fall hazards of approximately thirty (30) feet.    Windward Roofing & Construction, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.501(b)(1), which was contained in OSHA inspection number 912695, citation number 1, item number 1 and was affirmed as a final order on February 27, 2014, with respect to a workplace located at 105 E. Oakton Street, Des Plaines, IL. 60018.
