29 CFR 1926.453(b)(2)(v): A body belt with lanyard attached to the boom or basket was not worn by employee(s) while working from an aerial lift:    a) 2975 Richmond Avenue, Staten Island, NY 10314. Employees were exposed to fall hazards when working on and from a truck mounted aerial lift with no fall protection system in place.      Condition occurred on or about 04/06/15.         Dependable Electric Co., Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.453(b)(2)(v), which was contained in OSHA inspection number 315642256, citation number 1, item number 1 and was affirmed as a final order on 04/04/15, with respect to a workplace located at 2975 Richmond Avenue, Staten Island, NY 10314.
