29 CFR 1910.134(f)(2):  The employer shall ensure that an employee using a tight-fitting facepiece respirator is fit tested prior to initial use of the respirator, whenever a different respirator facepiece (size, style, model or make) is used, and at least annually thereafter:     a.  At the job site on the first floor and mezzanine area, employees who were wearing 3M 8511 NIOSH N95 dust masks while performing demolition activities were not fit tested on the filtering facepieces.

29 CFR 1910.134(g)(1)(i):  The employer shall not permit respirators with tight-fitting facepieces to be worn by employees who have:     29 CFR 1910.134(g)(1)(i)(A): Facial hair that comes between the sealing surface of the facepiece and the face or that interferes with valve function:     a.  At the job site on the first floor and mezzanine area, three employees who were required to wear 3M 8511 NIOSH N95 dust masks while performing demolition activities had beard growth that came between the sealing surface of the facepiece and face.

29 CFR 1926.102(a)(1):  Employees shall be provided with eye and face protection equipment when machines or operations present potential eye or face injury from physical, chemical, or radiation agents:   a.  At the job site on the first floor and mezzanine area, employees were not protected with safety glasses at a minimum while performing demolition activities such as using a reciprocating saw, thereby exposing employees to a struck-by hazard.
