29 CFR 1910.178(l)(4)(ii)(B):   Refresher training, including an evaluation of the effectiveness of that training, shall be conducted when the operator has been involved in an accident or near miss incident to ensure that the operator has the knowledge and skills needed to operate the powered industrial truck safely:  Employees operating a Yale tugger were involved in the following accidents and the employer did not ensure refresher training was provided to the employee following the accident:  a) On or about March 19th, 2015, an employee operating a Yale tugger was involved in an accident and suffered an injury to the left shoulder.   b) On or about March 27th, 2015, an employee operating a Yale tugger was involved in an accident and suffered an injury to the left foot.   Abatement certification is required of this item in accordance with the requirements of 29 CFR 1903.19(c).

Further notation of additional information may have been required depending upon the classification.  b) On or about March 27, 2015, an injury was recorded on the OSHA Form 300 as having occurred on the plant floor to the foot of a materials employee. The employer did not enter a check mark in column G, H, I, or J as required to classify the case. Further notation of additional information may have been required depending upon the classification.   c) On or about March 25, 2015, an injury was recorded on the OSHA Form 300 as having occurred on the plant floor to the right hand of a materials employee. The employer did not enter a check mark in column G, H, I, or J as required to classify the case. This case involved work restrictions necessitating a check mark in column I with the corresponding number of days of restricted work recorded in column L.  d) On or about March 19, 2015, an injury was recorded on the OSHA Form 300 as having occurred on the plant floor to the left arm of a materials employee. The employer did not enter a check mark in column G, H, I, or J as required to classify the case. This case involved work restrictions necessitating a check mark in column I with the corresponding number of days of restricted work recorded in column L.  e) On or about March 18, 2015, an injury was recorded on the OSHA Form 300 as having occurred on the plant floor involving back pain of a materials employee.
