29 CFR 1926.100(a): Employees working in areas where there was a possible danger of head injury from impact, or falling or flying objects, or from electrical shock and burns, were not protected by protective helmets:    a)    South side of the medical office building, 320 Route 73 North, Voorhees, NJ:  Employee without head protection was applying reinforcing mess and the base coat of the exterior insulation finish system for the medical office building under construction while working from the Skyjack model SJ 7135 scissors lift approximately 14 feet above ground level, on or about 04/09/2015;    a)    West side of medical office building, 320 Route 73 North, Voorhees, NJ:    Employees without head protection were applying reinforcing mess and the base coat of the exterior insulation finish system for the medical office building under construction while working from the two scissors lifts and a makeshift scaffold platform approximately 21 feet above ground level, on or about 04/09/2015

29 CFR 1926.451(b)(1)(i): Each platform unit (e.g., scaffold plank, fabricated plank, fabricated deck, or fabricated platform) was not installed so that the space between adjacent units and the space between the platform and the uprights was no more than 1 inch (2.5 cm) wide, except where the employer can demonstrate that a wider space is necessary (for example, to fit around uprights when side brackets are used to extend the width of the platform):     a)         West side of MOB, 320 Route 73 North, Voorhees, NJ: The makeshift used by an employee approximately 21 feet above the next lower level had a gap approximately 4-6 inches at the north end, while applying reinforcing mess and a basecoat, on or about 04/09/2015.    NOTE:  IN ADDITION TO ABATEMENT CERTIFICATION, THE EMPLOYER IS REQUIRED TO SUBMIT ABATEMENT DOCUMENTATION FOR THIS ITEM, FAILURE TO COMPLY WILL RESULT IN AN ADDITIONAL PENALTY OF $1000.00 IN ACCORDANCE WITH 29 CFR 1903.19.
