29 CFR 1926.651(c)(2): A stairway, ladder, ramp or other safe means of egress was not located in trench excavations that were 4 feet (1.22m) or more in depth so as to require no more than 25 feet (7.62m) of lateral travel for employees:  At this excavation site, at this excavation site, employees entered a vertical wall trench which was over 6 feet deep and 9 feet wide and 142 feet in length to install an 8 inch water pipe. There was not a ladder in the trench for easy access or egress from trench.

29 CFR 1926.652(b): The slopes and configurations of sloping and benching systems were not in accordance with the requirement options of 29 CFR 1926.652(b)(1), (b)(2), (b)(3), or (b)(4):  At this trench site, employees installing a 8 inch water line were not adequately protected while working in a trench which was inadequately benched. The trench was 142 feet in length, 9 feet wide and various depths from east to west beginning at 5.5 feet to 6.6 feet deep which exposed the workers to a possible trench collapse.
