29 CFR 1926.501(b)(11): Each employee on a steep roof with unprotected sides and edges 6 feet (1.8 m) or more above lower levels was not protected from falling by guardrail systems with toeboards, safety net systems, or personal fall arrest systems:    (a)  Jefferson, WV:  Employees were exposed to a fall of approximately 10 1/2 feet to the ground while cutting and sealing the curb for a deli fan on a 5/12 pitch metal roof, as determined on or about April 8, 2015.
