29 CFR 1926.501(b)(14): Each employee working on, at, above, or near wall openings (including those with chutes attached) where the outside bottom edge of the wall opening is 6 feet or more above lower levels and the inside bottom edge of the wall opening is less than 39 inches above the walking/working surface, was not protected from falling by the use of a guardrail system, a safety net system, or a personal fall arrest system.   a) 45-46 Fathers Way Guilderland NY. - On or about April 9th, 2015 - Employees were working on, at, above, or near wall openings where the outside bottom edge of the wall opening was approximately 10 feet or more above a lower level.  Employees were exposed to fall hazards of approximately 10 feet in height.
