29 CFR 1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems.   a. Construction Site - On or about April 9, 2015, employees were exposed to 10.7 feet fall while installing plywood decking for second story flooring.

29 CFR 1926.1053(b)(1): Where (a) portable ladder(s) was/were used for access to an upper level landing surface and the ladder's length did not allow the ladder side rails to extend at least 3 feet (0.9 m) above the upper landing surface being accessed, the ladder was not secured at its top to a rigid support that will not deflect, and/or a grasping device was not provided to assist employees in mounting and dismounting the ladder.    a. Construction Site - On or about April 9, 2015, the side rails of a wooden job made ladder did not extend 3 feet above the landing.  The ladder was used to gain access to a second floor where plywood decking was being installed.
