This condition exposed employees to possible struck-by hazards if a load were to fall off of the hook.     b) Back of shop, 1014 New England Hollow Road, Clairton, PA 15025:  Observed on or about April 08, 2015, employees were operating an electric chain hoist (3 ton capacity, serial number L-412HL, crane designation CR2) without a safety latch on the hook.  This condition exposed employees to possible struck-by hazards if a load were to fall off of the hook.

Employees using the bench grinder were exposed to caught-by hazards.      b) Front of Shop, 1014 New England Hollow Road, Clairton, PA 15025:  Observed on or about April 08, 2015, the work rest on the right wheel of a bench grinder was not adjusted such that the maximum opening between the abrasive wheel and the work rest was not more than one eighth of an inch.  The gap between the abrasive wheel on the right side of the bench grinder and the work rest was approximately three fourths of inch.  Employees using the bench grinder were exposed to caught-by hazards.      c)  Back of shop, 1014 New England Hollow Road, Clairton, PA 15025:  Observed on or about April 08, 2015, the work rest on a Kindt-Collins disc sander (model R, model number 6041) was not adjusted such that the maximum opening between the abrasive wheel and the work rest was no more than one eighth of an inch.  The gap between the abrasive wheel and the left side of the work rest (working area) was approximately one fourth of an inch.  Employees using the disc sander were exposed to caught-by hazards.

Employees using the coffee pots and nearby appliances were exposed to the hazards of burns and electric shock due to the missing outlet cover.    c)  Employee break room, 1014 New England Hollow Road, Clairton, PA 15025:  Observed on or about April 08, 2015, the light switch located in the employee break room was not equipped with a cover.  Energized conductors located in the light switch were exposed.  Employees using the light switch to operate the light in the employee break room were exposed to burns and electric shock.
