29 CFR 1910.95(b)(1): When employees are subjected to sound exceeding those listed in Table G-16, feasible administrative or engineering controls shall be utilized. If such controls fail to reduce sound levels within the levels of Table G-16, personal protective equipment shall be provided and used to reduce sound levels within the levels of the table:  (a) Frame - On or about April 15, 2015 the framer was/were exposed to continuous noise at 150.4 % of the permissible daily noise (eight hour time weighted average sound level of 90 decibels) or an equivalent sound level of approximately 93.0 decibels during the 350 minutes  sampling period on April 15, 2015.  Exposure calculations included a zero increment for the time not sampled.

29 CFR 1910.95(c)(1): The employer did not administer a continuing, effective hearing conservation program as described in 29 CFR 1910.9(c) through (o) whenever employee noise exposures equal or exceed an 8-hour time-weighted average sound level of 85 decibels measured on the A scale, or equivalently a dose of fifty percent:   (a) Rip Saw - On or about April 15, 2015 the rip saw operator was/were exposed to continuous noise at 91.8 % of the permissible daily noise (eight hour time weighted average sound level of 90 decibels) or an equivalent sound level of approximately   89.2 decibels during the 350 minutes sampling period on   April 15, 2015.  Exposure calculations included a zero increment for the time not sampled.  (a) Blower - On or about April 15, 2015 the blower operator was/were exposed to continuous noise at 66.5% of the permissible daily noise (eight hour time weighted average sound level of 90 decibels) or an equivalent sound level of approximately 87.0 decibels during the 350 minute sampling period on April 15, 2015.  Exposure calculations included a zero increment for the time not sampled.
