On or about April 9, 2015 and at times prior there to, the Geronimo line was not installed from the Derrickman's working platform. The employees were exposed to fire hazards.

On or about April 9, 2015 and at times prior there to, the employer failed to assess the need for personal protective equipment when employees were exposed to hazardous chemicals such as, but not limited to, Bentonite Gel and Caustic Soda. The employees were not provided with eye protection or chemical resistant gloves and were exposed to eye, skin, and lung burns or irritation.
