The Forest Service was previously cited for a violation of this occupational safety and health standard or its equivalent standard, which was contained in OSHA inspection number 978142, notice number 2, item number 1 and was affirmed as a final order on 12/20/2014, with respect to a workplace located at Bend Pine Administration Bend Oregon.  The Forest Service was previously cited for a violation of this occupational safety and health standard, which was contained in OSHA inspection number 106753,notice number 2, item number 1 and was affirmed as a final order on November 25, 2011, with respect to a workplace located at Halfway, Oregon.
