29 CFR 1910.305(b)(2)(i): Each outlet box in completed installations did not have a cover, faceplate, or fixture canopy: Saw Shack: On or about April 7, 2015 and times prior thereto, the Forest Supervisor did not ensure that the outlet box have a faceplate installed. This exposed employees to potential shock hazard from incidental contact with live electrical parts. The Forest Service was previously cited for a violation of this occupational safety and health standard or its equivalent standard, which was contained in OSHA inspection number 978154 , citation number 2, item number 4 and was affirmed as a final order on12/30/2014, with respect to a workplace located at Bend Pine Administration, Bend Oregon. The Forest Service was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1910.305(b)(2), which was contained in OSHA inspection number 312391923, notice number 2, item number 4 and was affirmed as a final order on 07/14/2010, with respect to a workplace located at Bly, Oregon.

29 CFR 1910.305(g)(2)(iii): Flexible cords were not connected to devices and fittings so that tension would not be transmitted to joints or terminal screws: Laundry room: On or about April 7, 2015 and times prior thereto, the Forest Supervisor did not ensure that the cord to the dryer have adequate strain relief. This exposed employees to potential shock/burn hazard if the connections touched each other grounding out the dryer.
