29 CFR 1926.1053(b)(1): Portable ladders were used for access to an upper landing surface and the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder was used to gain access: a)1120 Morris Tpke, Short Hills, NJ:Employee was exposed to falls of approximately 12 feet to the ground below as the ladder rails did not extend above the roof by three feet.        Violation observed on or about 4/11/15.

29 CFR 1926.1053(b)(5)(i): Non-self-supporting ladders were not used at an angle such than the horizontal distance from the top support to the foot of the ladder way approximately one-quarter of the working length of the ladder (the distance along the ladder between the foot and the top support: a)1120 Morris Tpke, Short Hills, NJ:Portable ladder did not have a set-up angle of about 75 feet, a 4-1 ratio of the ladder's working length to set-back distance.       Violation observed on or about 4/11/15.
