29 CFR 1926.1053(b)(16): Portable ladder(s) with structural defects, such as, but not limited to, broken or missing rungs, cleats, or steps, broken or split rails, corroded components, or other faulty or defective components, were not either immediately marked in a manner that readily identifies them as defective, or be tagged with "Do Not Use" or similar language, and shall be withdrawn from service until repaired:  A) 105 Highview Manor Drive, Woodbridge, NJ 07095: Employees were allowed to use a Louisville ladder to access the roof that had damaged cleats.  Condition occurred on or about April 13, 2015.

29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet or more above the lower levels was not protected by guardrail systems, safety net systems, personal fall arrest systems, nor was an alternative fall protection plan which meets the requirements of 29 CFR 1926.502(k) developed and implemented:     a) 105 Highview Manor Drive, Woodbridge, NJ 07095: Employees were observed performing roofing operations at a height of approximately 24' to the ground below with no fall protection in place, exposing them to fall hazards.    Condition observed on or about April 13, 2015.   Best Brothers Construction was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.501(b)(13), which was contained in OSHA inspection number 1055435, citation number 1, item number 2 and was affirmed as a final order on 08/05/2013, with respect to a workplace located at Bay Avenue & Campbell Street, Union Beach, NJ.
