29 CFR 1926.501(b)(3): Each employee in a hoist area was not protected from falling 6 feet (1.8m) or more to lower levels by guardrail systems or personal fall arrest systems:   On or about April 14, 2015, at a worksite located at 208 Springside Drive, Akron, OH 44333, during the hoisting and unloading of materials during final detail work, the employer did not ensure employees were protected from fall hazards ranging from 30 to 40 feet, by ensuring a fall protection system system/device was being used.

29 CFR 1926.1052(c)(1)(i): Stairways having four or more risers or rising more than 30 inches (76 cm), whichever is less, were not equipped with at least one handrail:   On or about April 14, 2015, at a worksite located at 208 Springside Drive, Akron, OH 44333, during construction activities including but not limited to finishing and final detail activities, the employer did not ensure employees using the existing stairways were provided at least one handrail on stairways having four or more risers. Employees were thereby exposed to falls while ascending and descending the stairways to the second, third and fourth floors to perform work activity.
