29 CFR 1926.451(g)(1): Employees on scaffolds more than 10 feet (3.1 m) above a lower level were not protected from falling to that lower level by fall protection established in paragraphs (g)(1)(i)-(vii) of this section: A) East side of building, on or about April 15, 2015, employees were installing siding on the side of a building, using a pump jack scaffold that did not have a mid-rail across the back of the scaffold and guardrails on either end, exposing the employees to fall heights of 10 to 18 feet.

29 CFR 1926.454(b)(2): Employees were not trained on the correct procedures for erecting, disassembling, moving, operating, repairing, inspecting, and maintaining the type of scaffold in question: A) East side of building, on or about April 15, 2015, employees who erected a pump jack scaffold were not properly trained on fall protection requirements and the proper erection of this type of scaffold, exposing the employees to fall hazards.
