29 CFR 1926.451(c)(2)(v): Fork-lifts were used to support scaffold platforms while the entire platform was not attached to the fork and the fork-lift was moved horizontally while the platform was occupied:    Altitude Roofing, LLC project site at 36 W. Washington St. Chilton, WI: On 4/14/15, an employee was working from a personnel platform supported by a Rough Terrain Forklift when the platform was not secured to the forks by the pins or other methods.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):     Altitude Roofing, LLC project site at 36 W. Washington St. Chilton, WI:On 4/14/15, two employees reroofing a two story house were not protected from 15 feet and greater fall hazard by utilizing personal fall arrest systems or any other acceptable form of fall protection.    Altitude Roofing LLC was previously cited for a violation of this occupational safety and health standard, which was contained in OSHA inspection number 988507,  citation number 1, item number 1 and was affirmed as a final order on 9/25/14, with respect to a workplace located at 421 Saratoga St. Chilton, WI 53014.
