29 CFR 1926.501(b)(4)(i): Each employee on walking/working surfaces was not protected from falling through holes (including skylights) more than 6 feet above lower levels, by personal fall arrest systems, covers, or guardrail systems erected around each hole:  a) 150 Avenel Street, Avenel, NJ 07001. Employees were exposed to fall hazards when the fall protection system that was installed was inadequate in that the horizontal cable lifeline system was not properly installed in that it was wrapped around sheet metal duct work on the roof.     Incident occurred on or about 04/15/15.

29 CFR 1926.502(b)(2): Midrails, screens, mesh, intermediate vertical members, or equivalent intermediate structure members were not installed between the top edge of the guardrail system and the walking/working surface when there was no wall or parapet wall at least 21 inches (53 cm) high:  a) 150 Avenel Street, Avenel, NJ 07001. Employees were exposed to fall hazards when an intermediate guardrail was not installed between the top edge of the guardrail system and the walking/working surface.      Condition occurred on or about 04/15/15.
