29 CFR 1926.453(b)(2)(iv): Employee(s) working in an aerial lift were sitting or climbing on the edge of the basket:  On or about 15 April, 2015, at the above addressed job site, an employee stood on the mid rail of the aerial lift as he leaned forward to place wood planks down on the roof of the building under re-construction, exposing the employee to a fall hazard of approximately 16 feet.

29 CFR 1926.453(b)(2)(v): A body belt with lanyard attached to the boom or basket was not worn by employee(s) working from an aerial lift:  On or about 15 April, 2015, at the above addressed job site, an employee working from an aerial lift was carrying wood planks on to the building under re-construction and did not utilize fall protection, exposing the employee to a fall hazard of approximately 16 feet.
