29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):  a)  On or about 04-15-15 on S. Commercial Street in Neenah, an employee was working at the edge of a roof approximately ten (10) feet above surface level without utilizing fall protection equipment.

29 CFR 1926.503(a)(2)(iii): The employer did not assure that each employee has been trained, as necessary, by a competent person in the correct use and operation of personal fall arrest equipment:   a)  The employer did not provide adequate fall protection safety training to his employees including but not limited to the proper donning of a personal fall arrest system and equipment use requirements.  Employees who had personal fall arrest systems available for use were not inspecting the equipment prior to use and did not adjust the harness for the safe use location of the chest strap and D-Ring.

Commercial Street in Neenah, an employee was utilizing an extension ladder to access/egress a roof surface at approximately nine and one half (9.5) feet above surface level with the side rails extended less than two (2) feet above the upper landing surface.
