29 CFR 1903.19(d)(1): The employer must submit to the Agency, along with the information on abatement certification required by paragraph (c)(3) of this section, documents demonstrating that abatement is complete for each willful or repeat violation and for any serious violation for which the Agency indicates in the citation that such abatement documentation is required:  a.  The employer has failed to submit abatement certification and documentation within ten days of the abatement date, July 1, 2015, for a violation of 29 CFR 1910.67(c)(2)(iv), issued as citation 1, item 1, and described as follows:      In the elevated basket of the extensible boom platform, the employee climbed out of the platform to gain access to the cable, thereby being exposed to an 18 foot fall hazard.  b. The employer has failed to submit abatement certification and documentation within ten days of the abatement date, June 25, 2015, for a violation of 29 CFR 1910.67(c)(2)(v), issued as citation 1, item 2a and described as follows:    In the elevated basket of the extensible boom platform, the employee was not protected from falls by the use of a harness and lanyard, thereby being exposed to an 18 foot fall hazard.  c. The employer has failed to submit abatement certification and documentation within ten days of the abatement date, June 25, 2015, for a violation of 29 CFR 1926.132(a), issued as citation 1, item 2b, and described as follows:    The fall protection lanyard that was in the truck available for the employees to use was unserviceable and no longer in a reliable condition, in that the deceleration device of the lanyard was deployed, exposing employees to fall hazards.

Protective equipment, including personal protective equipment for eyes, face, head, and extremities, protective clothing, respiratory devices, and protective shields and barriers, shall be provided, used, and maintained in a sanitary and reliable condition wherever it is necessary by reason of hazards of processes or environment, chemical hazards, radiological hazards, or mechanical irritants encountered in a manner capable of causing injury or impairment in the function of any part of the body through absorption, inhalation or physical contact.   a.
