29 CFR 1926.451(b)(1): Each platform on all working levels of scaffolds shall be fully planked or decked between the front uprights and the guardrail supports as follows:  a. On the west side of the house where employees were working from the scaffold, the scaffold was not fully planked, thereby exposing employees to a 14 foot 2 inch fall hazard.

29 CFR 1926.451(c)(2): Supported scaffold poles, legs, posts, frames, and uprights shall bear on base plates and mud sills or other adequate firm foundation.  a. On the west side of the house where employees were working from the scaffold, the mudsills that were used were pieces of scrap wood and were not adequate, thereby exposing employees to a struck-by, crushed-by, or fall hazards.

29 CFR 1926.451(g)(1): Each employee on a scaffold more than 10 feet (3.1 m) above a lower level shall be protected from falling to that lower level. Paragraphs (g)(1)(i) through (vii) of this section establish the types of fall protection to be provided to the employees on each type of scaffold. Paragraph (g)(2) of this section addresses fall protection for scaffold erectors and dismantlers. Note to paragraph (g)(1): The fall protection requirements for employees installing suspension scaffold support systems on floors, roofs, and other elevated surfaces are set forth in subpart M of this part.   a. On the west side of the house where employees were working from the scaffold, the ends of the scaffold were not guarded, thereby exposing employees to a 14 foot 2 inch fall hazard.  b. On the west side of the house where employees were working from the scaffold, cross braces were installed with no midrails, thereby exposing employees to a 14 foot 2 inch fall hazard.
