29 CFR 1910.243(c)(1): Abrasive wheel(s) were used on portable grinder(s) which were not provided with safety guard(s) meeting the requirements specified in 29 CFR 1910.243(c)(1) through (c)(4):            (a):    On or about March 29, 2015, at the above addressed worksite, employees were using a DeWalt angle grinder without a guard, exposing the employees to the hazards of being struck-by broken wheel fragments in the event of failure.            (b):    On or about March 29, 2015, at the above addressed worksite, employees were using a Chicago Electric angle grinder without a guard, exposing the employees to the hazards of being struck-by broken wheel fragments in the event of failure.

29 CFR 1910.305(b)(1)(i): Conductors entering cutout boxes, cabinets, or fittings were not protected from abrasions:        (a):    On or about March 29, 2015, at the above addressed worksite, cables used as extension cords for a Millermatic 252 MIG Welder and a Plasma Cutter were directly wired into two bottom openings of an outside electric cabinet, exposing employees to the hazards of electrical shock and/or burns.

29 CFR 1910.305(g)(2)(ii): Hard-service cord and junior hard-service cord No. 14 and larger were repaired by splicing, which did not retain the insulation, outer sheath properties, and usage characteristics of the cord that was spliced:        (a):    On or about March 29, 2015, at the above addressed worksite, a 50amp/250volt plug was sliced onto the end of a hard-service cord with electrical tape and connected to a 4-way metal gang box, exposing employees to the hazards of electrical shock and/or burns.
