29 CFR 1926.452(k)(2): Ladders used to support the ladderjack scaffolding did not meet the requirements of Sub-part X.    a) Building Exterior - Front Roof Left: Employee was exposed to a fall hazard of approximately 22 feet while engaged in roofing activities from a ladder jack platform supported by an aluminum extension ladder that had a missing first rung , on or about 4/16/2015.
