29 CFR 1926.1053(b)(1): Because of the portable ladder's length, the ladder side rails did not extend to at least 3 feet (.9 m) above the upper landing surface to which the ladder was used to gain access and the ladder was not secured at its top to a rigid secure point. (a) Mini Storage Building, Oxford, MS. - On or about April 16, 2015, the ladder used by employees to access the roof did not extent 3 feet above the landing thereby exposing employees to a fall hazard.

The employee utilized the ladder to fasten metal roofing.
