On April 10, 2015, employees using of construction tools such as skill saw, drills and a wheelbarrow to carry a wet cement mixture were not required to wear safety-toe footwear, exposing the employees to foot injuries.

On April 10, 2015, a written Hazard Communication Program (HCP) was not developed for employees using hazardous materials on the construction project to include, but not limited to Portland cement, paints and Acrylic Block Sealer.
