29 CFR 1910.22(a)(1): All places of employment, passageways, storerooms, and service rooms shall be kept clean and orderly and in a sanitary condition.  a. South side of warehouse located at 4311 Janitrol Road, Columbus, Ohio: On or about April 20, 2015 and at times thereof, the passageway to the emergency eye/shower station and fire extinguisher area was obstructed by a parked forklift and a loose water hose, exposing employees to trip hazards.

Warehouse located at 4311 Janitrol Road, Columbus, Ohio: On or about April 20, 2015 and at times thereof, the employer did not remove a Raymond Order Picker Forklift, serial EASi-96-BD16650, from service due to damaged guard rails.

Sufficient access and working space shall be provided and maintained about all electric equipment to permit ready and safe operation and maintenance of such equipment.  a. Battery charging station at the rear of the warehouse located at 4311 Janitrol Road, Columbus, Ohio: On or about April 20, 2015 and at times thereof, adequate access and working space was not provided at nine 480 volt electrical disconnects and a 120/208 volt electrical panel located directly behind the forklift battery charger racking for employees performing service and maintenance.
